Amy Jordan is an AANP board-certified Family Nurse Practitioner and the founder of Western Roots Healthcare, a telehealth practice serving individuals across Oregon. Her career in healthcare spans more than 25 years, beginning with a decade in the clinical laboratory and followed by 15 years as an emergency department RN before becoming a Family Nurse Practitioner. Telehealth A telehealth visit is a video call with your healthcare provider, like a virtual doctor’s visit on your screen. At your appointment time, you click the secure link that is sent to you and your clinician appears on video so you can talk face to face.​ Equipment Needed All you need one device that has a camera, a microphone, and internet, such as a smartphone, tablet, laptop, or desktop computer. Make sure your device is charged, connected to Wi‑Fi, and in a quiet, private place so you can hear and speak clearly. Making Appointments New patients: Click the "Request appointment" button, a secure form will open for you to chose which type of appointment you want with a date and time then a form with open requesting your contact information. Existing patients: Log in to your Client Portal first, then click "Request appointment" for more appointment options. You will get an email when your visit is approved, with easy steps to join your video visit. Telehealth visits can be scheduled online up to 2 weeks ahead and as late as 24 hours before the visit. If you need a time that is not listed (same-day, next-day, or more than 2 weeks away), please send a message or call Western Roots Healthcare with the day and time you want, and the office will work with you to set this up. Once your appointment is confirmed, you’ll receive an email with: a link to download the telehealth app, required visit forms, and a questionnaire about your medical history and medications. Please complete all forms before your appointment so more of your visit can focus on you and your concerns. You will receive a link to your appointment for the telehealth visit. Billing Care is kept simple and focused on you. Credit or debit card information is entered when you schedule your appointment, and charges are processed after midnight on the day of your visit. We are currently in the process of becoming credentialed with major insurance plans. Transparency is important to us and payment details will be outlined prior to your appointment. You may still use your insurance for approved radiology studies, lab work, and medication. Coverage varies by insurer and medical condition. We do accept Medicare Part B at this time. A detailed receipt (superbill) will be provided within 24-48 hours of your visit so you can submit it to your insurance company for possible reimbursement. This streamlined approach ensures your time is spent on your health and goals—not paperwork or delays. Cancellations If you need to cancel or change your telehealth visit, please do so at least 24 hours before your appointment time. Changes made less than 24 hours in advance will automatically be charged in full by the system unless manually deleted by the provider before the visit occurs. Important Legal Details Patients must be Oregon residents and physically located in Oregon at the time of their appointment, as care is provided under state licensing regulations. Questions?
